Lower Blepharoplasty | Jonathan Britto Clinic

The lower eyelid is a small but highly complex aesthetic and functional unit. What patients call ‘eyebags’ may be caused by several different structures, and successful treatment begins with precise analysis.

At the Jonathan Britto Clinic, the emphasis is on evidence-based excellence, layer-by-layer analysis and a natural result that remains individual to the patient. Here are ten of the questions people commonly ask when researching this procedure in London.

1. What is lower blepharoplasty?

Lower blepharoplasty, or lower eyelid lift surgery, is a family of procedures designed to restore a natural balance between the eye, lower eyelid and cheek. Jonathan describes lower eyelid aesthetic balance in terms of smooth, uniform reflection of light from the lower lid to the cheek, while preserving natural creasing in dynamic smile.

2. What causes under-eye bags and shadows?

‘Eyebags’ describe several different anatomical problems. Bulging fat, hollowing, skin laxity, lower eyelid position, the lid-cheek junction and festoons can all create shadows that make the eyes appear tired. The correct treatment depends on understanding which structures are producing the shadow.

3. Can lower blepharoplasty remove eye bags permanently?

Lower blepharoplasty can provide a long-lasting correction by repositioning or managing the tissues responsible for the bags and shadows. It cannot stop ageing, and the tissues continue to change with time. The aim is durable restoration of lower eyelid and cheek balance rather than simple removal of fat.

4. What is transconjunctival lower blepharoplasty?

Transconjunctival blepharoplasty is performed through the inside of the lower eyelid, so there is no external skin scar. It is a powerful but limited option suited to particular anatomical problems. Patients with skin laxity or other structural issues may require a different or extended technique.

5. Will lower blepharoplasty change the shape of my eyes?

The aim is to preserve or restore natural eye shape, not alter it unnecessarily. Lower eyelid surgery demands careful judgement because lid position and support are fundamental to both appearance and function. Jonathan’s oculoplastic experience informs the analysis of these structures.

6. Can lower blepharoplasty improve dark circles?

It can improve dark shadows when they are created by contour irregularity between the lower eyelid and cheek. Pigmentation or very thin skin may have different causes and may not be corrected by surgery alone. The consultation distinguishes shadow from colour and determines realistic treatment goals.

7. How long is recovery after lower blepharoplasty?

Jonathan’s treatment information indicates that lower blepharoplasty is usually daycase surgery, commonly under local anaesthetic or ‘Twilight’ conscious sedation. Patients are generally socially presentable early in recovery, with return to work often around 10 to 14 days and a soft, stable result around six weeks.

8. Where are lower blepharoplasty scars?

The incision depends on the procedure. A transconjunctival approach leaves no visible scar on the lower eyelid skin. Where an external incision is required, it is carefully positioned close to the lash line or natural creases and planned to heal as discreetly as possible.

9. Can I have lower blepharoplasty if I have had it before?

Yes, revision lower blepharoplasty may be possible. Secondary surgery requires particularly careful analysis of what the first operation achieved, what has changed and the condition of the remaining tissues. Risks, alternatives and expectations need to be discussed comprehensively.

10. Can lower blepharoplasty be combined with facelift or browlift surgery?

Yes. The eyelids, cheek, brow and face are neighbouring aesthetic units, and combination surgery can be appropriate when ageing changes affect more than one area. Jonathan’s aim is for these areas to remain naturally balanced in repose and emotional expression.
