What Causes Loss of Definition in the Upper Eyelids?

Upper eyelid drooping or loss of definition can result from two main causes: intrinsic fat excess and intrinsic skin/muscle excess or laxity.

Skin Excess: In some cases, an excess of skin and muscle accumulates within the eyelid, much like a set of “Venetian blinds” that drop down over the eyelid crease This causes the upper eyelid to lose its natural shape and definition, giving the appearance of heaviness and contributing to tired, aged-looking eyes.

Intrinsic Laxity: In other cases, the eyelid does not have an excess of skin and muscle but simply experiences a loss of elasticity. This results in the skin and muscle falling down over the eyelash line, similar to “a loose pair of socks around the ankles.” The solution here isn’t to remove skin or muscle but to restore and elevate the upper eyelid crease, ensuring stability and a refreshed look.

Why Is Upper Blepharoplasty the Solution?

Upper blepharoplasty works to redefine the eyelid by addressing both the volume and functionality of the eyelid. For those with excess skin and muscle, the procedure involves careful removal of tissue, allowing for better definition of the eyelid crease. However, in cases of laxity where there is no excess, the procedure focuses on elevating and reshaping the eyelid for a more natural, youthful look without compromising the ability to close the eyes comfortably.

Importantly, removing excess skin or muscle from a lax eyelid without proper planning can create complications, including difficulty with eyelid closure, hollowing of the eye and the

While ageing is a common cause for eyelid changes, it’s not the only factor. Ethnic differences also play a role in upper eyelid shape and definition. For instance, in some Oriental eyelids, there may be a desire to create an upper eyelid skin crease known as a “double fold eyelid,” which gives the eyelid a more defined lid show commonly seen in Caucasian eyelids.
